A variety of services are necessary because individual need, as determined by health status, finances, and other factors, vary greatly among people who require LTC services. LTC services should: fit the needs of different individuals, address the changing needs over time, and suit people’s personal preferences. Services should focus on:  Individualized services  Well-coordinated total care—requires that any health care need is recognized, evaluated and addressed by appropriate clinical professionals. Case management is a key coordinating function.  Promotion of Functional Independence—goal is to maintain functional independence to the masximum level possible and practicable (ADLs and IADLs)  Extended Period of Care  Holistic Care  Quality of Life 2. Age is not the primary determinant for long-term care. Comment on this statement, explaining why this is or is not true. The primary determinant for long-term care is the inability to be functionally independent. Long-term care is for those who need assisted care to promote maximum possible independence for people with functional limitation. Primary recipients of LTC are elderly, but this does not mean that most elderly people are in need of such care. In 2008, 91% of elderly either lived alone or with a spouse. 77% of adults ages 65 or over assess their own health status as good to excellent. LTC clients are mostly elderly, but include children adolescents with developmental disabilities or mental retardation, younger adults with permanent disability, and people with HIV/AIDS. 3. What is meant by “quality of life?” Briefly discuss the five main features of this multifaceted concept. Quality of life is the sense of satisfaction, fulfillment, and self-worth and is regarded as critical patient outcomes in any health care delivery setting. Significance of the quality of life in the long term care setting arises from the loss of self worth that accompanies disability and the relative amount of time that patient remain in this setting without the prospect of full recovery.  Lifestyle factors: personal enrichment and meaning through activities one enjoys  Living environment: must be comfortable, safe, and appealing to the senses
